Privacy Policy

From 21 December 2001 we have applied the 10 Australian National Privacy Principals, as set out in the Privacy Act 1988, to the way we collect, use, store and disclose your personal and sensitive information.

We continue to follow regulations established under the Assisted Reproductive Treatment Act (1988) SA, and we participate in the national accreditation process conducted by the Reproductive Technology Accreditation Committee (RTAC). Patient files are viewed during RTAC site visits, and all members of the visiting team sign a confidentiality agreement.

Collection, Use and Disclosure of your Information

We use patient information to assist us in improving the quality of care we give to patients. Your personal health information will not be disclosed to a third party unless you have consented to this, or it is relevant to public health or public safety, or where denying access would be unlawful. Disclosure of information is necessary to obtain Medicare payments and other health insurance rebates. We are also required to report assisted reproductive technology treatments and pregnancy data to ANZARD (Australia and New Zealand Assisted Reproductive Database). This information (when collected) is reviewed by RTAC.

Data Quality

Flinders Reproductive Medicine will ensure that the personal information it collects, uses or discloses is accurate, complete and up-to-date.

Data Security

Flinders Reproductive Medicine will protect the personal information it holds from misuse and loss, and from unauthorised access, modification or disclosure.

Openness

The Flinders Reproductive Medicine Privacy Policy is available in both electronic and printed form:

  • on the Flinders Reproductive Medicine website www.flindersivf.com.au
  • at Flinders Reproductive Medicine, Reception, Level 4, Flinders Medical Centre, Flinders Drive, Bedford Park, SA

Access and Correction

Flinders Reproductive Medicine will provide individuals
with access to their information on request by the
individual, except to the extent that:

  • providing access would pose a threat to public health or
    safety; or
  • Providing access would have an unreasonable impact
    upon the privacy of other individuals; or
  • The request for access is frivolous or vexatious; or
  • Providing access would be unlawful; or
  • Denying access is required or authorised by or under law.

Identifiers

Flinders Reproductive Medicine assigns its own identifiers, being a unique number to identify individuals.

Anonymity

Wherever it is lawful and practicable, individuals have the option of not identifying themselves.

Transborder Data Flows

Flinders Reproductive Medicine may transfer personal information about an individual to someone in a foreign country only if:

  • The recipient of the information is subject to a law; or
  • The individual consents to the transfer; or
  • The transfer is for the benefit of the individual.

Sensitive Information

Flinders Reproductive Medicine will not collect sensitive information about an individual unless:

  • The individual has consented; or
  • The collection is required by law; or
  • The collection is relevant to public health or safety.

Complaints

Please discuss concerns, questions or complaints about the privacy of your personal information with your doctor, the Unit Director or the Unit Privacy Officer.
